FACULTY BACKGROUND CHECK PROCEDURES


 

  1. Hiring Department submits request to interview to the appropriate Dean's Office
  2. Dean approves and submits interview recommendations to Equity Issues Office
  3. Equity Issues approves interview request and returns approved paperwork to appropriate Dean
  4. Dean notifies Department Chair of approval to interview
  5. Department Chair contacts candidates for interview and explains Background Check Consent Form must be signed prior to interview. 
    1. Background Check Consent Forms can be mailed, faxed, or e-mailed to the candidate.  Candidates can also download the form from the following website:  http://www4.semo.edu/humanresources/forms.
    2. Candidates can submit completed Background Check Consent Form to Department Chair via mail or fax.  If faxed, the original must be mailed or presented at time of interview.  Department Chair retains completed Background Check Consent Form with search materials in the interviewee's folder.
    3. Candidates cannot be interviewed until the original signed form is received by Department Chair.

                                                               i.      EXCEPTION – If interview candidate cannot complete the Background Check Consent Form, they must submit a written explanation.  Once written explanation is received, a consultation will take place between the Dean and the Provost to determine if the candidate can be brought to campus for an interview.

    1. If an interview is conducted without a signed Background Check Consent Form, the Department will be responsible for all travel expenses.
  1. Interviews are conducted.
  2. Department Chair submits request to hire to Dean for approval
  3. Dean approves and submits request to hire to Equity Issues
  4. Equity Issues approves hire and submits all paperwork to the Provost.  Provost Office verifies that all interviewees completed Background Check Consent Form.
  5. Provost approves the hire and sets salary
  6. Department Chair extends offer to candidate
  7. Candidate accepts offer
  8. Department Chair contacts Provost Office and contract letter is generated
  9. Provost Office sends completed Background Check Consent Form of new hire to Human Resources
  10. Human Resources processes Background Check
